Let me tell you a little about myself so you can get to know me.
Personally, I consider myself a family man because I love to spend time with my wonderful wife, Tanya, and our 5 children. Some of the things that bring me joy in my free-time include basketball, camping, fishing, beekeeping, gardening and do-it-yourself projects around the house. If you can’t find me doing any of those things, then I am somewhere creating things out of wood, clay, plastic or metal. My personal goal is to learn how to do as many things as I can and help my family to do the same.
Professionally, I will admit that we physical therapists can lose our focus on the patient and your priorities and resources because of the complexities of running a traditional healthcare business. I designed my physical therapy services to be simple so the focus can stay on getting you back from stop to go®. Firstly, I come to your home to help you in a natural environment where you spend your time. Because it is your home, it is a built in reminder that I have been invited into your life to help you and not to tell you how to live your life. Secondly, I use treatments that are simple and work without requiring specialized equipment. Lastly, I structured my payment system so I don't lose focus on getting you better faster.
I intend to keep my focus on you, so please, let me help you with your injured or misbehaving bodies that are keeping you from finding or fulfilling your purposes in life.

In 2013, I graduated from the University of Puget Sound with a Doctor of Physical Therapy degree; In 2009, I graduated from the University of Utah with a bachelor's degree in Exercise Science with a minor in Nutrition
Working at Various Roles in the World of Physical Therapy
I better understand what patients want and need having filled these roles: patient, technician, office manager, Spanish interpreter, student, staff doctor, family coach, clinic director and practice owner
Doctor to Patient Assessment and Treatment Encounters
After helping patients of all ages from infants to the end of life at clinics, hospitals, nursing homes and sport sidelines, I found it most rewarding to treat people at home surrounded by their favorite people
You will immediately receive a confirmation email for the scheduled appointment. The confirmation email includes a link to your medical history and consent to treat form that must be filled out before your appointment. It also includes the cancellation policy and a reminder that payment is required at the time of the appointment by cash, PayPal, credit card or HSA card. You will receive a reminder email both 30 hours and one hour before your appointment.
For you to get the most of your treatment sessions, you are expected to:
Clear and tidy a space in your home or community location that will allow us to assess your response to various movements and activities free of pets and other distractions.
Wear comfortable, modest clothing that will allow you to move.
Be honest and ask questions to take an active role in understanding your condition.
Reschedule if you are sick (fever, flu, covid or other contagious illness)

No referral is needed to be examined and treated by Dr. Bohman. He is happy to help those who do have specific referrals from other medical providers despite it being unnecessary.

You will be required to pay the flat-rate fee at the time of your appointment by cash, PayPal, credit card or HSA card

*The only additional payment you may ever be responsible to pay is the "late-cancel/ missed appointment" fee or an "extended travel time" fee if you are outside the Tooele Valley and West Valley City service area
Once your appointment is booked, future appointments for other clients on the same day are approved or declined in relation to your location or appointment. Out of respect for our provider and our other clients, we ask that you give us at least 24 hours notice if you need to cancel an appointment.
If you as the patient miss your appointment without providing a notice at least 24 hours in advance, you will be charged a $20 fee. If your provider has to cancel the appointment less than 24 hours in advance you will receive a $20 discount on the rescheduled appointment.
If you live outside the Tooele Valley or West Valley City service area, you will be charged an additional travel fee
The fee is equal to $0.60 per mile of driving from Deseret Peak High School in Tooele, Utah
*Example: You want to be seen at your home near Murray Park which is 36 miles from Deseret Peak High School so your Travel Fee = $0.6(36mi) = $22
Our self-pay physical therapy can save you a significant amount of money by reducing the number of visits you need. We use the Mechanical Diagnosis and Therapy (MDT) model, which empowers you to perform the majority of your treatments at home. This approach leads to fewer in-person visits and a lower total cost for your care compared to using your insurance, particularly if you have a high deductible.
Please be aware that neither Dr. Kevin Bohman nor From Stop To GO® LLC are contracted with any health insurance carriers. We are an out-of-network provider for all insurance plans and cannot assist with any insurance reimbursement claims or paperwork.
You are welcome to use your Health Savings Account (HSA) to pay for our services.
If you are a Medicare or Medicaid beneficiary, we cannot accept your self-payment or provide physical therapy services at this time. H.R. 4204, also called the Medicare Patient Choice Act is a bipartisan bill that was introduced to Congress on June 26th 2025 and is meant to give you the option to self-pay for services. To explore other options like receiving wellness care, you must speak directly with Dr. Bohman at (435) 303-4678.
